Accommodation and Transportation
Important: - Each delegate needs to fill this form individually.
Perfect Travels & Tours Pvt. Ltd.., being the official travel agent for ISHTM 2010, will be providing special discounted rates for hotel rooms of all categories as well as transport services (transfers, tours, excursion).
Please complete this form and send it to us by E-mail / Fax. Hotel accommodation is subject to availability and cannot be guaranteed merely by filling this form.
Rooms will be subject to availability and will be allotted on first come first served basis. After receipt of your form duly filled by you, booking will be processed and we would revert to you in next 48-72 working hours.
Check-in and Check-out time is 12 noon. However, should there be an early arrival or a late departure and you would like us to allocate the rooms upon arrival of the delegates, we will arrange the accommodation, subject to availability, based on the following Hotels terms & conditions (Case to Case basis)
Payment policy of Accommodation:
i) 1 Night Payment at the time of booking**
ii) Balance Payment before 30 Sep’2010

Cancellation policy of Accommodation:
i) Full refund before 14 Sep ’2010
ii) No refund w.e.f. 15 Sep’2010.
All government policies/regulations prevalent during the time of your conference/ event will apply. All above hotels will not be held responsible for any subsequent changes in the Government policies, rules and/or regulations which will/may have a bearing on the Terms and Conditions of our agreement. In such an event, the Terms and Conditions mentioned herein in this contract will remain valid.
**Remark: - Deposit of one night accommodation is to be made upon receipt of hotel confirmation through us immediately by following mode of payments:-
a) By demand draft in favor of Perfect Travels & Tours Pvt. Ltd., payable at New Delhi and send it to Perfect Travels office address in New Delhi.

b) Directly deposit the cheque / telegraphic transfer (TT) onto our bank account whose details are given below under necessary intimation to Perfect Travels office thru email or fax:-
